    I'm actually going to go for either the 1/7th of the world that doesn't have access to clean water or sex trafficking. Not sure which, though.

    Water is a strong contender, with about half of the world's population supplied by potable sources

    Malaria is another (~500,000,000 cases worldwide, iirc)

    I can't think of anything that comes close to these two issues

    Besides access to clean water, I'd say access to and management of food resources and medicine/medical care.

    If we're talking more about the developed world, I suppose poverty and homelessness as well as the US criminal justice system.

    Distribution of wealth. People who have more than they ever need vs people who have nothing to even survive in the extreme case on a global level, but it is an important topic in every country.
