Yes, insurance companies do ration healthcare, but they actually do it less (albeit less efficiently) than most European healthcare systems. The US has the highest rate of healthcare expenses per capita in the world, and has the most capital-intensive healthcare system, as well as the highest life expectancy (adjusting for fatal accidents) in the world, as a result.
