So why don't you go see a specialist? It does sound like you really need serious help, of the non-informal/non-forum variety.

I really have no idea but it hurt my shoulder so much I blacked out.

How am I possibly supposed to cope with anything if I can't even do this? This is the question.

The answer is actually very straighfoward, you just have to do whatever you can. This'll take effort and will.

It's been mentioned above.. you need to see some professional help if you haven't already, and actually take their advice. You've mentioned before that this is not something you are apt to do.

Beyond that, if end up having a condition that can't be handwaved away, you need to learn what your limits are exactly, and see if you can push them. By this, I mean, see how far you can go without damaging yourself or being dangerous to others. Damage is not the same as pain, they're different things.

I've had a couple of back surgeries, maintain an active life, and deal with nerve type pain on a regular basis, so I'm pretty familiar with pain and physical limitation.

But the problem is that to push myself I would actually need to be conscious to do it...

You've stayed conscious long enough to keep yourself alive this long...

If you can do that you can do this.

The question is whether you actually want help or want to better your condition?

From where I sit it looks like you just like to bitch about it.

First: Professional help. Next: Work within your limitations, as defined by what you've learned from consulting a professional.

    You're jumping ahead.

Not really, no, because I have never actually been hurt before ever. I mean ever. Seriously. Barely even so much as a fucking paper cut.

I need to help myself because professionals won't.
